Membership Agreement

Upon enrolling, a member will receive a Membership Agreement that is a contract between VantageAmerica Solutions, Inc. and the member. The Membership Agreement details all the terms and conditions of the plan the member chose.

The member will also receive a membership kit. This kit will include ID cards and a membership handbook that will detail each of the benefits in their particular program.

Programs may not be available in all states, and not all services are available in all areas. Members must use participating Plan providers. Programs are not insurance. The doctor discount program cannot be used in conjunction with a Medicare eligible expense.

VantageAmerica Solutions, Inc. cannot assume responsibility for the actions or performance of professional providers.
